چکیده

Method The results of 831 operations with CABG in patients with CAD in 3 groups were evaluated: CABG in patients without diffuse lesions of coronary arteries (group I 711 patients), isolated CABG in patients with advanced CAD (group II 33 patients) and CABG combined with TMLR in patients with advanced CAD (group III 87 patients). Data evaluation was made on the basis of the data of intraoperative EchoCG and coronaro-shuntography, the values of cardiac enzymes (CPK and CPK-MB) and data of intraand postoperative evaluation, which allow valuing of patients condition.

Impact of Transmyocardial laser revascularization on Pathomorphological and Physiological Patterns of Myocardial Microcirculation in patients with advanced CAD

Method We investigated vessels of coronary microvascular network of patients with ENDCAD and analyzed the histological changes in two groups: in group of patients died after sole CABG (1-st group 8 heart specimens) and group of patients died after combined operations CABG+TMR (2-nd group 6 heart specimens). These data are compared with hospital results (deaths, MACE) in two large groups of patie...

Improved patient outcomes when transmyocardial revascularization is used as adjunctive revascularization.

BACKGROUND Transmyocardial revascularization (TMR) has been recently used to treat patients with angina for whom angioplasty/stenting and/or coronary artery bypass grafting (CABG) is no longer an option. METHODS A retrospective review of 255 consecutive patients who required CABG was done. Group 1 patients (n = 219) underwent complete revascularization with CABG alone. Group 2 patients (n = 3...

From controlled trials to clinical practice: monitoring transmyocardial revascularization use and outcomes.

OBJECTIVES We sought to examine trends in the use and outcomes of transmyocardial revascularization (TMR) in community practice. We also identified important risk factors for TMR and compared outcomes of TMR combined with coronary artery bypass graft surgery (TMR + CABG) versus bypass alone in patients receiving incomplete revascularization. BACKGROUND Although it is approved for use as a sta...

Complex Coronary Artery Disease

With recent increase in percutaneous cardiac intervention (PCI) the patients undergoing coronary artery bypass (CABG) are getting more complex with other medical problem. [1] In some patients standard surgical or percutaneous intervention is no longer available due to its complexity. We define complex coronary artery disease (CAD) as condition not amenable to percutaneous coronary intervention ...

How does caffeine increase exercise capacity but decrease myocardial flow reserve?

My fellow authors and I appreciate the comments expressed in Dr. Horvath’s letter, which largely claim that the sole reason for the negative results demonstrated in our study (1) relates to procedural differences between surgical versus percutaneous transmyocardial laser revascularization (TMR). Although we acknowledged in our study that full-thickness transmyocardial laser channels at surgery ...
